Overview

Due to an internal promotion we have an exciting opportunity for a full-time Unit Coordinator at The Huntington at Nashua.  The Huntington at Nashua is a gorgeous life plan/continuing care retirement community (CCRC) in Southern New Hampshire and is part of Silverstone Living.  This position is 40 hours per week, full-time and supports Kent Village with Nursing schedules as well as charting duties and reception tasks.

Responsibilities

  • Enter resident data into admission register.
  • Audit records of new admissions according to established criteria to assure completeness, accuracy, and compliance with state and federal regulations.
  • Ensure timely scanning of all medical records including but not limited to physician orders, treatments, progress notes, discharge summaries, transfer forms and any other data relating to the medical history and treatment of residents.
  • Audit discharged resident records according to established criteria to assure completeness, accuracy, and compliance with state and federal regulations.
  • Monitor frequency of physician/NP visits and notify physician/NP when they are due for visits.
  • Maintain schedule of due dates for residents’ annual physical exams and episodic visits.
  • Secure physician signature on all materials regarding medical judgment, such as orders, history and physical exam, discharge summaries, etc.
  • Maintain and control the release of information to authorized persons.
  • Track and monitor LNA and Nurse licenses and send out notifications to renew 30 days prior to expiration.
  • Track and ensure contracts are up to date.
  • Answer telephones for nursing.
  • Scheduled resident appointments and coordinate transportation.
  • Nursing staff scheduling (monthly) and assuring staffing needs are met.
  • Work with Resident Services to manage podiatry, foot clinic, and flu clinic/vaccination appointments.
  • Carry out additional duties as needed.
  • Assist with staffing schedule as requested to find coverage.
  • Assist in central supplies ordering and putting away, ordering office supplies.
  • Assist Director of Resident Services in entry of independent medical records into Matrix system.
  • Meet all new residents review orientation packets.
  • Coordinate employment and fire drills to ensure compliance.
  • Assist with monthly Memos to residents and families.
  • Keep family contacts updated .
  • Process billing for attends, O2, and transportation billing.
  • Coordinate best option for resident pharmacy needs, mail order, triage pharmacy concerns and assist with resolution.
  • Assist staff with transport, meal delivery, as needed.
  • Coordinate transfer of residents to new physician.
  • Assist nursing ordering labs work, x rays, making appointments.
  • Audit Physician orders.
  • Coordinate care planning as directed by the Director of Nursing.
  • Carry out additional duties as needed.

Qualifications

LNA License preferred. Data entry in electronic medical records or computer skills and previous experience in a health care setting with knowledge of medical terminology is preferred.  The Unit Coordinator must have the ability to work independently, organize the workload and communicate with physicians, nursing staff and allied health personnel.  The Unit Coordinator must maintain resident confidentiality.  He/she will ensure the accurate and timely compilation and scanning of all data relating to the medical records of all residents living in the community and will perform a variety of clerical duties.
